But anytime you get people new to big software implementations this happens. It happened with MRP, ERP and now EHR. People need to expect the worst but we are terribly naive and overly optimistic. This is the Fallacy of Planning.]]></description>

This is a disruptive transformation made worse by the culture in healthcare. Usability is a moving target. Physicians who sign-off on usability at implementation will have different needs as their skills improve or once their realize operational use was not as expected. And usability for a physician on rounds is way different than for the floor nurse.
